ATHLETIC CLEARANCE:
Here are the documents you will need to start the athletic clearance process:

Athletic Clearance Medical Documents
To start the Athletic Clearance process, go to www.AthleticClearance.com and follow the steps. During this process, in the medical section, you will see a "Download Forms" link. Once you print them out and complete them, you will need to upload them to your account.
*Be sure to UPLOAD and SAVE the following:
- Pre-Participation History form
- Physical Exam form
- Proof of Insurance
- Signed Confirmation Page.

Log in from your phone. Go to the Medical section and you will see "uploads." You can take a picture or upload it from your photo gallery. Once you are done, be sure to click on "save" and wait for the page to automatically open to the next screen. Then, you will know that it is successfully uploaded.
